This voluntary form allows the Cincinnati Metropolitan Housing Authority (CMHA) to share personal data—such as your contact info, lease status, and eviction notices—with Strategies to End Homelessness (STEH) to help track housing trends and offer eviction prevention services. Signing is completely optional, requires you to be at least 18 years old, and will not affect your CMHA housing or program status. While the agencies use strong security measures, data sharing carries a small risk of breaches, and signing does not guarantee you will receive financial help. You can cancel this agreement in writing at any time, and STEH cannot pass your information to other partner agencies without a separate consent form.
